Dentinogenesis imperfecta is a genetic disorder of the development of the tooth. This disorder causes the tooth to discolour. It occurs during histodifferentiation stage. Dentinogenesis imperfecta os a localized mesodermal dysplasia that was described in the 19th century and affects dentition.It is a type of dentin dysplasia and makes the teeth appear yellow-brown or blue-gray. Abnormal dentine structure characterizes dentinogenesis imperfecta. Teeth becomes weaker due to this condition. Also, teeth are disposed of to rapid wear, loss and cracking. Teeth appears translucent and thus the condition gives the teeth opalescent sheen root canals and the pulp chambers may be completely or partially obliterated.enamel shows hypocalcified or hypoplastic defects. This is the condition that is called Osteogenesis imperfecta.( Simmer et al,2007,p 392-399) Dentinogenesis imperfecta can affect both the permanent and primary teeth.
There are three types of this disorder. The first type arises for individuals with osteogenesis imperfecta while the second and the third type occurs in individuals that do not have inherited disorders. Loss of hearing is associated with this condition. Dentinogenesis imperfecta results to abnormal dentin formation. Dentinogenesis imperfecta type two has similar clinical histological and radiographic features as Dentinogenesis imperfecta type one without osteogenesis imperfecta. Dentinogenesis imperfecta type three is infrequent and found only among triracial Brandywine inhabitants of Maryland. Dentinogenesis imperfecta three and one result from different expressions of the similar gene. Dissimilar types of Osteogenesis imperfecta occurs due to mutation of COLIA 2 and COLIA 1 genes.( Levin et al.,1983, p.267-274)These genes are responsible for encoding the pro-alpha 2 and 1 polypeptide chains of the collagen type one.patients with Osteogenesis imperfecta suffer from a deficiency in growth, asthma, and Dentinogenesis imperfecta.
Dentinogenesis imperfecta is a clinically heterogeneous and only affect tooth or teeth.abnormal dentin enamel junction causes the crowns to fracture with ease while the exposed dentin suffers rapid attrition.The tooth affected by this condition exhibit short and thin roots.there is also constriction in the cervical portion of the tooth.Osteogenesis imperfecta has similar features to those of Dentinogenesis imperfecta. (Witco et al., 1988, p 547-553)Dentinogenesis imperfect also affects connective tissue. Dentinogenesis imperfecta causes hypoplasia wich is a defect that leads to alteration of the teeth shape that results in localized loss of the enamel.
Dentinogenesis imperfecta type two is inherited in automals simple dominant mode.Early treatments and diagnosis of Dentinogenesis imperfecta are necessary because this measures may lead to prevention of teeth deterioration, and improvement of the esthetic value of the teeth.treatment is done in two stages name stage one and two.In stage one, incisors are covered with a composite restoration.In addition, the first primary molars teeth are covered with preformed crowns.This treatment takes place around the early age of eighteen to twenty months.the second stage of this treatment pursues to protect the second molars.( Shields et al.,1973,p 543-553)This is done by covering them with preformed crowns.the canines are covered with composite restorations in the second stage wich is done at around an age of twenty-eight to thirty months
The aim of earlier treatments are;
* Preserving and maintaining form, vitality and the size of dentition.,
* The esthetic appearance that leads to prevention of phycological issues.
* Prevention of damage of vertical dimensions.
* It helps in the maintenance of arch length.
* Normal growth of temporomandibular joint and facial bones is facilitated.
Stainless crowns made of steel,overdenttures, removable appliance and steel crowns that contain acrylic facing are used for treatment. Indirect resin crowns are used in the restoration if the maxillary incisors.this are used in the treatment of patients with extreme enamel attrition and hypoplasia.In less extreme cases, Orthodontic treatment may be applied.Discolor problem may be eliminated using bleaching agents such as carbamide peroxide.restoring treatment recommends the use of glass ionomer with chemical attributing materials and fluoride release.composite restoration is used for restoring anterior affected teeth.In addition, polycarbonate crowns provide substitutes for restoring primary anterior teeth.
Early treatment may result in problems.This problems include;
* Risk of anesthesia
* Restoration of teeth that have minimal clinical crowns with wide pulp chambers may result in technical problems.
* High treatment cost
However, there are advantages of earlier treatments.Its advantages overpower the disadvantages, i.e.,
* Prevention of possible damage of the vertical dimension.
