Jennifer Jackson is a female, 24 years old. He was born in Mansfield, Texas. She is a Native American. She is 5.3 ft tall. About her marital status; she is married.
Past Health History
In the last five days, Jennifer has been experiencing burning feeling in the stomach. This burning feeling has been occurring mostly between meals or during the night. The sternness of the pain in the patient’s stomach can be rated at 7 (on a scale of 1-10). Pain is only felt in the stomach, but it does not increase. Jennifer noticed burning feeling in hers stomach after taking lunch five days ago. About Jennifer Past Health: her general health has been good, infancy sickness includes severe chicken pox and chronic bronchitis, Jennifer has never been injured.
Family History
Having been married for two years, Jennifer had been pregnant once. She gave birth seven months ago, with the method of delivery being vaginal. During the delivery, there was no any complication.
Review of Systems
About the constitutional symptoms: Jennifer has experienced mysterious weight loss. Eyes are operating normally; she normally has chest pain, and lately she has food avoidance.
