The American airline 191 crashed minutes after taking off. Witnesses at the airport said they observed a white smoke come from the proximity of the number one engine structure. The crash occurred because of unintended structure damage. Unlike the maintenance team's assumption, the pylon object and the engine were supposed to be removed as a unit. The maintenance team made those assumptions because removing the parts was not an approved method. Additionally, the lack of precise instructions on using forklift during reinstallation of the pylon resulted in the damage of the structure (Wind et al., 2021). Finally, quick inspections of the maintenance routine could not have allowed the inspection team to detect or identify a problem.
The maintenance and inspection team performed their tasks incorrectly before the plane took off. FAA surveillance technology overlooked issues with the unusual maintenance process.
