Competence refers to the attributes of an individual which mainly results to an effective performance. Some standards were developed for the practice which was known as the competency standards (Australian Nursing Council 2005). The Australian Nursing and Midwifery Council Incorporated (ANMC), is the national nursing and midwifery organization that was started in 1992. the main purpose of this body was to develop a national approach to the nursing and midwifery regulations. This essay aims at assisting the nurse student attending a five week placement in the antenatal and gynecology ward, to understanding the concepts and strategies of the practice domains in the ANMC Competency Standards.
The competencies that make up the ANMC National Competency Standards for the Registered Nurses working in the antenatal and gynecological ward can be categorized into four domains. The Competency standards of the nurses were developed using a range of research techniques and the findings were that practice of the advanced enrolled nurse is characterized by a number of learning objectives. The first character is that the nurse has to be specialized in an area or field of practice. The nurse is also expected to have acquired a higher level of clinical knowledge and skills which will call for a greater level of responsibility in the management of client cared within this ward (ANMC 2005). Drawing on these findings, the four domains of the advanced enrolled nurse that were developed and they include the professional practice, critical Thinking and Analysis, Provision and Coordination of Care, and finally the Collaborative and Therapeutic Practice.
The professional practice domain reflects its competence through the nurse’s active engagement in the ongoing practicum period in the antenatal and gynecology ward. This nurse should be able to satisfactorily demonstrate know ledge on all stages of antenatal care and also the proper ways of child delivery in the gynecological wards. The nurse should be able to demonstrate a lot of accountability for their action during the time when they will be under practice (Gillis 2007). It is very necessary for the registered nurses to always act in accordance to with the legislation that is directly related to the nursing and health care issues with the aim of protecting human life.
As a nurse under practicum in the antenatal and gynecology department, it is important for one to coordinate nursing and health care of these patients in order to ensure that they are able to get optimal health outcomes (Borbasi 2009). Every registered nurse can only be able to efficiently provide health quality care committing themselves towards getting a professional development, generating research data and also through clinical supervision as well as developing some clinical and practice guidelines.
According to the professional code of professional conduct, it would be very necessary for the nurse working in the antenatal and gynecology ward to remember that they will always be held responsible for their actions towards the patients under them. It would be right for the nurse to acknowledge any limitation of competence towards any duty allocated to them before taking any action (Chenoweth et al 2006). In some instances, the nurse may not be familiar with some gynecological complications of even abnormal behavior of the infant after delivery among other situations. It would only be considered professional, for the nurse to wait until they can receive instructions from their administrators on how to handle certain problems rather that administering the wrong prescription.
The practicing nurse should also be conversant with the Critical thinking and Analysis domain. Its main concept is to get professional development in the line of nursing practice through self appraisal. The nurse should always remember that any consequences of their acts will directly affect the patient’s health and in this case it will either be the life of the mother or that of the unborn child.
It is the responsibility of the nurse under practice in the antenatal gynecology ward, to participate in trying to bring about professional development in the ward and clinic in general, through the introduction of new theories and methods of solving problems within this ward. This will be brought about by having thorough research and analysis of the critical matters being done by the nurse under practice (Australian Nursing Council 2008). Participating in healthy analytical and research programs is one way that can help the nurse in giving in their contribution in professional development of themselves and others working in the antenatal ward and gynecology. The expansion of practice can also be done but only with thorough consideration of the legislation, national policy as well as the guidelines. The nurse should also be able to determine if they really are competent to carry out the role of midwifery in accordance with the standards provided by the ANMC.
The domain on the Provision and co-ordination of care for the practicing nurse in the antenatal gynecology ward for five weeks, tries to focus its objectives in coordination, organization and the provision of nursing care to the patients in these wards. Competencies in this domain mainly focus in having the nurses providing care to individuals and groups having complex needs either within the antenatal gynecology ward or outside the health care setting with a greater degree of indirect registered nurse supervision. As a registered nurse, one should be able to meet mandatory competencies of undertaking ongoing professional development and annual performance review.
The national competency standards for the midwife nurse, was the criteria through which the performance of the nurse was analyzed for one to retain the license of operation. Midwives wanted to be viewed to have a clear professional distinction from the nurses. A midwife is a person who has successfully completed the midwifery educational program that is duly recognized in the country where it is operating (Stuart 2007). One is expected to be able to offer all the necessary supervision, care and advice to expectant mothers undergoing their antenatal care, and preparation for parenthood. This care also extends to some gynecological conditions of the patients in the wards.
The fourth domain of the ANMC competency standards is the collaborative and therapeutic practice .This domain is mainly related to sustaining and concluding the professional relationships with the individual patients. It is the role of the collaborative and therapeutically pre-higher authorities to get concerned with what services the nurse under practice in the antenatal gynecology ward will be offering to the patients. The nurse should be able to portray a lot of competence in providing the health care services in the antenatal and gynecological ward. The nurse should maintain a constant communication line with the interdisciplinary health care team so that they can be sure that they are on the right note, throughout the time when they will be under practice in the clinic (ANMC 2005).
The individual and Midwifery practice in the antenatal and gynecological ward is mainly defined by the midwifery professions and registration procedures. Some of the duties of the nurse within this profession will be to carry out pregnancy test and monitor the growth and development of the embryo. The mother is supposed to attend monthly clinical observations to ensure that they have normal pregnancies ongoing. The registered nurse is expected to carry out all the necessary examinations and with the earliest detection of a pregnancy risk; they are expected to report this to the clinical administration then wait to get instructions from them (Queensland Health 2005).
For competency to be achieved, the nurse undergoing the practice in the antenatal ward is also expected to introduce a program of parenthood in complete preparation for childbirth.
