Bradford Diabetes Reduction Program (BDRP) Student’s Name Institution Affiliation Course Name Professor’s Name Date Bradford Diabetes Reduction Program (BDRP) Summary The Bradford Diabetes Reduction Program (BDRP) is an initiative aimed at reducing the number of diabetes cases in Bradford, which has recently been recorded as having the highest number of diabetes cases in the United Kingdom. The program has been effective in reducing the number of diabetes cases in other parts of the UK and around the world, and it is hoped that similar results will be achieved in Bradford. The planning phase of the program involves the participation of experts such as local government officials, dieticians, general practitioners, Public Health England, and community members in Bradford. The underlying theory behind the development of the program has been fully described. The BDRP will be delivered by a few public health nurses who will be trained to provide the therapy to patients over the course of the program. The BDRP involves cognitive therapy focused on the healthy lifestyle that patients should adopt to reduce their risk of developing diabetes or to prevent diabetic complications if they already have diabetes. Assessment Diabetes is a serious public health issue because it is a chronic disease that affects millions of people worldwide and has resulted in serious health complications when not managed properly. In 2019, there were approximately 4 million people living with diabetes in the UK, and it is estimated that a further 1 million people have undiagnosed diabetes (National Health Service, 2020). The number has been increasing, and the issue is quite concerning. The cost of treating diabetes and its complications is high and can place a significant burden on individuals, families, and the healthcare system. Many cases of diabetes can be prevented through lifestyle changes, and there is a need for increased public awareness about diabetes and its risk factors, as well as improved access to preventive and management services. According to the National Health Service (2020), diabetes is a major cause of morbidity and mortality in the United Kingdom. Diabetes is the leading cause of blindness in people of working age in the UK and a major cause of kidney failure, amputations, and cardiovascular disease. In 2018, approximately 22,000 people in the UK died from diabetes-related causes. The cost of treating diabetes and its complications is also significant, with estimates suggesting that diabetes accounts for around 10% of the NHS budget (Public Health England, 2018). These statistics highlight the significant impact of diabetes on mortality, morbidity, and the overall health and well-being of individuals and communities in the UK. The National Health Service (NHS) has implemented a diabetes policy to improve the care and management of individuals with diabetes (National Health Service, 2020). The policy aims to ensure that all patients with diabetes have access to high-quality care and support to manage their condition effectively. Under the policy, individuals with diabetes are offered an annual review with a healthcare professional, which includes an assessment of their diabetes control and any complications they may be experiencing. They are also provided with a personalized care plan, which includes advice on diet, physical activity, and medication management. Bradford is the region in the United Kingdom with the highest number of people diagnosed with diabetes. The city requires an immediate solution to this rising cases of diabetes, with Bradford having more than 44,000 people diagnosed with diabetes (Ganner, 2021). This translates to 8.9% of the population in Bradford, which is a high number of people in a region suffering from diabetes. Moreover, considering that the rate of diabetes in England is approximately 6.4% of the total population in England (Ganner, 2021). There will be one GP, a representative from Public Health Bradford, a dietitian, a representative from local government, and a member of the Bradford community involved in the project. All of these stakeholders will play an important role in the project's completion and also in enabling them to achieve all of the goals. Planning The intervention taken will be to adjust the lifestyle of people in Bradford. Poor lifestyle has been a major cause of diabetes and diabetes-related complications in Bradford and England as a whole. Poor lifestyle behaviors, such as lack of physical activity, poor diet, smoking, and heavy alcohol consumption, can contribute to the development of diabetes (World Health Organization, 2019). Physical activity helps to regulate blood sugar levels and improve insulin sensitivity, so a lack of physical activity can increase the risk of developing diabetes. Many people in the city hardly have time for any physical activities due to busy schedules. Moreover, these busy schedules lead to high consumption of fast foods that are high in unhealthy fats, added sugars, and refined carbohydrates, which can contribute to weight gain and increase the risk of developing diabetes. Behavioral management of diabetes can lead to improved health outcomes. Cognitive behavioral theory (CBT) is a psychological theory that has been applied to the management of diabetes to help people change behaviors that are related to the management of their condition. CBT focuses on the role of thoughts and beliefs in shaping behavior and aims to change problematic behaviors by changing negative thoughts and beliefs. In the context of diabetes management, CBT may be used to help people overcome barriers to self-management, such as fear of needles or lack of motivation to follow a healthy lifestyle. CBT interventions may include techniques such as goal setting, problem solving, and relaxation techniques to help people adopt behaviors that support the effective management of their diabetes sugar control. Behavioral changes can be an effective intervention in the management of diabetes in Bradford district. The main reason why the intervention remains effective at reducing the diabetes rate is because poor lifestyle has been the main cause of the high rate of diabetes in the district. Therefore, behavioral change interventions can help individuals develop healthy habits, such as regular physical activity and a healthy diet, which can improve blood sugar control and overall health. Behavioral interventions are an important component of a comprehensive diabetes management plan, which helps individuals better manage their condition and improve their quality of life. However, behavioral changes do not work for everyone, as some individuals may not be receptive to behavioral change interventions or may not have the necessary skills or resources to successfully implement the recommended changes. When patients do not have sufficient resources, they may miss important parts of the intervention, making it less effective. In these cases, it may be necessary to consider alternative approaches or to work with the individual to identify and address any barriers to success. Another shortcoming is that access to behavioral interventions can be a challenge for some individuals with diabetes. There may be limited availability of these interventions in some areas, or the individual may not have the financial resources to pay for them. Therefore, this may exclude some people when the program is implemented in the community. However, the government can chip in and help make the program easily accessible and inexpensive in order to ensure that most people in the community receive it and its intended goals are achieved. Nonetheless, these obstacles only impact a few people in the community, and the majority of people have a positive reception to behavioral changes. A systematic review and meta-analysis of behavioral interventions for adults with type 2 diabetes found that these interventions led to statistically significant improvements in HbA1c (a measure of blood sugar control) and other outcomes (Shah et al., 2021). Therefore, with such a high rate of positive impacts in the community, the intervention is highly reliable in the management of diabetes in Bradford district I would like to describe how I will be developing my intervention protocol. The following are steps that I will follow when developing an intervention protocol. 1 Identifying the behaviors that are the target of the intervention. These behaviors should be specific, measurable, achievable, relevant, and time-bound (SMART). 2 Determining the target population for the intervention. This may involve defining the age range, gender, and other characteristics of the individuals who will be included in the study. 3 Selecting appropriate intervention strategies. This may involve choosing between different types of interventions, such as group or individual sessions, or selecting specific techniques or approaches (e.g., cognitive-behavioral therapy, motivational interviewing). 4 Developing the intervention materials. This may involve creating handouts, videos, or other resources that will be used during the intervention. 5 Testing and refining the intervention. It is important to pilot test the intervention to ensure that it is feasible, acceptable, and effective, and to make any necessary adjustments before implementing it more widely. 6 Implementing the intervention. This may involve recruiting participants, training staff, and delivering the intervention in accordance with the protocol. 7 Evaluating the intervention. It is important to assess the effectiveness of the intervention and to identify any areas for improvement. The implementation stage will be a 10 phase stages as described below. 1 First stakeholders meeting (first month) 2 Pre-implementation (0-2 month) 3 Initiation (3-12 months) 4 Early adoption (12-18 months) 5 Late adoption (18-30 months) 6 Maintenance (30-42m...
