Identification of potential barriers to plan implementation, and a discussion of how these could be overcome.
Appendix section, if tables, graphs, surveys, educational materials, etc. are created.

Background
The current medical practice is overwhelmed by a rise of lifestyle conditions and diseases such as diabetes, cancer and obesity. However, an analysis of diabetes indicates that the condition if left untreated it can lead to heart failures, blindness, kidney failure, amputation and even death. severity of the diabetes is conditioned by non-compliance by the patient. As such, different socio-economic factors cause the rise of diabetes and non-compliance in medical treatment leading to severity of the infection (Taylor et al., 2019). These factors include; poverty, cultural influences, religious and peer influences.
Clinical Problem Statement
Uncontrolled diabetes is the most prevalent clinical challenge while considering treatment for patients. patients suffering from diabetes fail to comply to the prevention and treatment programs leading to the development of their infections. As such, while the hospital setting is motivated to treat and educate patients upon discharge the fail to follow the guidelines in prevention of diabetes (Peer et al., 2020). Therefore, to what degree is self-care effective in patients with uncontrolled diabetes mellitus.
