Introduction
Theories are constructed and developed by nursing theorists to describe and explain phenomena in nursing. Nursing science and development of the profession of nursing relies on this process to expand the understanding of what nursing is and what nursing does. The purpose of this competency assessment is to explore how a theorist explains their philosophy and thoughts and then develops their theory based on testing and use in practice. An evaluation of the theory will make clear its usefulness to nursing practice, leadership, and/or education.
Directions
In this assessment, you are going to explore how a nurse theorist of your choice created their theory. You will become the theorist immersing yourself in the writings from the earliest mentioned to the most current literature. Writing in the voice of the theorist (taking on the persona of the theorist), you will create the theory through the following four stages.
In the first stage, theorizing occurs. This is where you, as the theorist, identify the concepts of what nursing is and is not. Perhaps you questioned what ideas were guiding those in nursing practice and then started to question your role. You started asking yourself, “Where am I in nursing, and where the profession is going? Is there some overarching understanding that guides the professional in their practice?” This is where you recognize that a theory is needed.
In the second stage, syntax is developed. This is where you will define the terms, explain relationships between the terms, and examine their expansion. Syntax development is an evolution. Consider, for example, Jean Watson. She starts by defining the word “caring.” Within the last several years, she has refined her terminology changing the term caring to caritas and identified the relationships between terms. This demonstrates a growth and maturation from decades of research she and other scholars did to produce the theory of caring.
The third stage is theory testing. Here, scholars and researchers consider whether this theory helps answer questions that arise in nursing. This is where your theory is examined and tested in research studies. For example, graduate nursing students request to use the tool you developed while testing your theory in an area of nursing.
The fourth and last stage is evaluation. This is where the theory is used in practice with the goal of improving healthcare. It is evidenced in the development of policies, procedures and best practice standards that have evolved from theory implementation.
Assessment Details
Begin your paper with an introductory paragraph that describes why you chose this theorist. First person voice (I, me, etc.) is acceptable in this paper.
Write in the first person voice (I, me, our, etc.) as the theorist. For example: I, Dr. Jean Watson, found that caring was a core concept of how I viewed nursing. To me, caring is the essence of nursing.
As the theorist, use the four stages (theorizing, syntax, theory testing, and evaluation) to explain how your theory was developed and used in nursing.
In the first (theorizing) stage, provide the historical context that influenced the theorist’s thinking.
In the second (syntax) stage, describe the development of the concepts and statements.
In the third (theory testing), provide two (2) examples of research examining the theory.
In the fourth (evaluation), provide three (3) examples of how the theory has been used in practice and provide a brief evaluation of the theory.
Conclude your paper in either the voice of the theorist or your own voice with a summary of the important points.

Nursing theories provide frameworks that nurses should adopt in caring for patients. The theories define the concept of nursing, what the practitioners should do, how they should and why they should adopt the practice. The theories confirm that nursing is a unique discipline that is different from other professions like medicine. I, Dr. Madeleine Leininger through my observation while working as a nurse established that lack of cultural and care knowledge when supporting patients was a missing component to foster effective care. I noted that it is important for nurses to have cultural knowledge about the patients and incorporate it into the care plan and practices. I thus developed the culture care theory to guide the nurses on effective care of patients through the incorporation of the patients’ cultural values and beliefs in the care plans.
Theorizing Stage
Nursing is a broad concept with different meanings based on the theorist. I, Dr. Leininger, perceive nursing as a component of care that should pay attention to the values, beliefs and preferences of patients with the aiming of bringing healing. I, therefore, believe that nursing is not about the use of general approaches to support clients, but rather, the utilization of patient-specific cultural components such as norms, values and ideologies to provide care (McFarland & Wehbe-Alamah, 2017). My development of the culture care theory was informed by the need to challenge the poor practices being observed by nurses already in practice. While working as a nurse, I established that a significant portion of nurses was utilizing the general approach to care for patients. They thus lacked the revelation that patients come from different cultures that indicate their mode of care.
My development of the theory was aimed at challenging their understanding of nursing and bringing the best concept to guide their practice. I established that most of the nurses view the nursing process as supporting patients to realize their desired healing. Even though this understanding has some level of truth, it lacks the component of culture that dictates how patients should be cared for in the healthcare facility. McFarland and Wehbe-Alamah (2019) opine that cultures have beliefs, norms and values that nurses should establish and incorporate in care practice. Also, patients resisted the care provided by nurses since it was not aligned with their culture. Therefore, I, Dr. Leininger, established the need to guide nurses in shifting from the general nursing approaches to specific techniques that are guided by the preferences and value-system of the patients. The utilization of culture care theory will, therefore, eliminate resistance to care by some patients considering that they will be involved in the design of care plans with their values bring integrated into the care processes.
Syntax Stage
Nursing is a practice and a process of realizing the best clinical outcomes. In the first stage of theory development, I, Dr. Leininger, defined nursing as a practice that pays attention to the care of patients, with the nurses expected to engage in activities that help, support and facilitate individuals to regain their health (Foronda, 2020). I also perceived nursing as a process of providing support to people to enable them to face handicaps or death. My continued interaction with patients from different cultures enabled me to establish that culture dictates the care that the clients should receive. I, therefore, modified my definition of nursing and examined it as a process of care that stress the incorporation of cultural beliefs, values and practices in the care of patients (Steefel, 2018). I noted that patients are only comfortable with nursing when the process is tailored around their culture, values and health-illness context. Thus, I redefined the process to capture the concept of culture and its influence on the wellness of individuals.
Theory Testing
Several studies have been conducted to establish the effectiveness of my culture care theory in realizing positive clinical outcomes. One such study was conducted by Listerfelt, Fridh and Lindahl (2019) who sought to find out the experiences of nurses in critical care in caring for patients from culturally diverse communities. The researchers conducted 4 focused group discussions that involved 15 nurses. Listerfelt et al. (2019) found that nurses with cultural knowledge of different communities had an easy time supporting patients to realize their wellness considering that they incorporated the cultural demands in the care plans. On the contrary, Listerfelt et al. (2019) established that nurses lacking inter-cultural competencies experienced difficulties in caring for patients. These findings are aligned with my arguments in the culture care theory where I advocate for intercultural knowledge on the part of the nurses.
Transcultural nursing education programs have aided in best support for patients, thereby confirming the effectiveness of my culture care theory. Another study was conducted by Tosun et al. (2021) to find out the usefulness of transcultural nursing education in equipping nursing students with competencies for caring for patients from different cultures. The research involved the review of other studies conducted on the topic. The authors established that nurses trained on cultural diversity have positive experiences caring for patients from different cultures. The argument provided by the authors is that such nurses know the cultural aspects that influence the choice of care plans and therefore incorporate these aspects in the design of care plans (Tosun et al., 2021). Also, the authors established that patients responded positively to treatment plans that incorporated their preferences, values and beliefs. These findings confirm that my culture care theory is effective in realizing wellness.
Evaluation
Nursing practice does not only begin in healthcare centers but from the nursing schools where the nurses are trained on effective care of patients. My culture care theory is practiced in both nursing schools and care settings. In the latter, nursing schools are adopting intercultural training as a practice within the nursing discipline. Nurse trainees are subjected to ethno-nursing practice where they are trained on values, beliefs and practices of different cultures (Antón-Solanas et al., 2021). The training allows them to effectively care for their patients. Secondly, the theory is practiced in the care of patients. There is the concept of patient-centered care that require nurses to design care plans aligned with the values, beliefs, customs and references of the patients (Hakansson-Eklund et al., 2019). The approach also allows the patients to participate in the decision-making practices regarding their care plans. Finally, my culture care theory is applied during the health promotion campaigns. The culturally competent nurses advise the populations of the target to adopt the best dietary choices, physical activities and a lifestyle that is aligned with their cultures while ensuring that their choices have positive health outcomes. My culture care theory is effective in improving the quality of care while also fostering positive clinical outcomes.
