1. Mental Status Examination:
MSE is a way in which the patient is reported according to what is observed by the psychiatrist. It is usually a description on what and how the patient reacts and responds to the interview. The aim of mental status examination as indicated by (Groth-Marnat, 2009) is to identify the patient’s situation and find out if at all he is suffering from mental problem or not
When pulled out of the river Hemi was fully dressed in jeans and a hoodie, when placed under an interview by the police, he doesn’t respond well to the questions asked, rather he is less interested with the interview being carried on and only converses with himself. In his responses to the interrogation by the crisis worker, Hemi provides confusing details. He claims that there existed a kora that persuaded him to try and cross the river and that he communicates with it (Cormier, Nurius, & Osborn, 2009). He has become a matter of concern with his parents in that they consider him a changed person from what he used to be before. In the video hemi is speaking to himself and throughout the interview he mentions kora.
2. Present a Clinical Formulation:
It’s an approach that combines bio-psychosocial factors. Information is viewed as either presenting, predisposing, precipitating, perpetuating and protective. They are all included in the clinical presentation of the patient by the psychiatrist (Haynes & O'Brien, 2011).
Presenting factors are the major and initial concern of the patient. They assist in ensuring that one has a clear understanding concerning the patient. Hem’s situation is as a result of the death of his kora, hence, Hemi’s change in behaviour. They claim he has never been the same gain since the death of his grand mother who has been attributed to being very close to Hemi, thus, she is the route cause of the situation.
The other factors are predisposing Factors. In our case the immediate surrounding has influenced the situation that Hemi is experiencing. The fact that he has experience a change in the environment in that the fact that his kora will never be around again has not been accepted by his mind in that he feels as if she is around.
Perpetuating Factors are those that are hindering Hemi from feeling better. His parents may be the reason as to why his situation is like. Although they are concerned they have played a lesser role in ensuring that Hemi comes in terms with the death of his Kora, they look confused on the situation of Hemi thus becoming a hindrance in the recovery of their son.
The last factors are the protective factors in which they assist in the well being of Hemi. They include the environment in that change of location away from where Kora lived may change the situation of hemi since it will help in erasing the memories of kora. The other component is the parents who should play a supportive role and issue him with new hopes and by doing so hemi will settle psychologically. From both the mental and clinical formulation a set of approval and diagnoses is determined to ensure a solution for the whole case is found (Ingram, 2011).
3. Maslow’s Hierarchy of Needs
The hierarchy of beings is commonly a pyramid of ones need. From the low end of the pyramid are the basic need which includes clothing shelter and food. In our case Hemi was found fully dressed and with the availability of his parents all basic needs seemed to have been provided to him. From the basic needs the second one in the hierarchy is the safety need, this may be in terms of financial, personal and health and well being of an individual. Hemi in our case seems unsecured with his health and well being in that he considers speaking to non existent person. From the safety needs follows the social needs (Rosdahl, 2008) in which all is about belonging. Hemi has lacked a sense of belonging in that he considers communicating with the outside world and has lost control of his body both mentally and physically.
The pyramid is then followed by the self esteem needs; they include the confidence of a person to carry out certain matters in life. Hemi’s case has seen him loose his self esteem among his parents and surrounding. In that they all consider him not as Hemi but as one whom is affected mentally. Lastly is the self actualization, they are all about a persons confidence and realization of the full potential in that hemi is perceived to be affected but there is a chance to reverse the situation (McGuire, 2012).
Hemi has acted in manner that he may cause injuries to himself and is affected in that he speaks to an imaginary Kora. The professions then choose two major needs which in the case of Hemi are all about self esteem and self actualization. In the idea of ensuring that Hemi regains his confidence back and the perception in the society changes back to how it used to be.
4. Recovery Model
Hemi is suffering from self esteem and actualization and hence it is important to note that they are to be dealt with in the bid of rescuing the patients him from his dreamland. In ensuring that it’s effectiveness the model relies on hope, connectedness, personal responsibility, active sense of self and discovery. There exists many ways of ensuring this is effective and no single method is identified to be the most effective (Johnson, 2008).
While dealing with the Hemi’s case, hope plays a major role in ensuring his recovery in the clinical practise in that working with him in ensuring his positives are shared. By ensuring personal connectivity in the clinical set up hemi needs to be involved in that questions asked are to be build on the basis of how the patient is being handled that is does he feel connected to the clinical officers.
