The chapter explores midwifery care as an integrated or contested privilege. It examines disparate maternity care systems, including provider types, midwifery education and maternal choice. Besides, it locates midwives in the United States and the Netherlands by outlining unique historical trajectories, obstacles to integrated care, and the impact on outcomes in the United States and the Netherlands. Davis-Floyd and Cheyney (2019) also discuss childbirth paradigms in the context of the two countries coupled with history, the biosocial nature of birth, and working across paradigms.
