Introduction
To preserve consistency and patient safety, nurses must use their skills and knowledge in inpatient care. All patients have the right to high-quality, cost-effective, and fair nursing care. According to the WHO, nurses must adapt to changing patient needs to be effective in today's environment. Aside from that, nurses must add evaluation procedures to determine therapeutic efficacy. Nursing care is crucial in today's environment, with diseases like diabetes rising. According to the Diabetes Organization UK's 2016 annual report, over 5.5 million people have diabetes. There are millions of undiagnosed and diagnosed diabetics worldwide. Diabetes is a global disease impacting over 600 million individuals. Diabetes prevalence has increased by about 8% in low and middle-income nations like Turkey compared to high-income countries like the UK (World Health Organization, 2016). Due to the high cost of diabetes management, low- and middle-income countries suffer enormous economic losses. Nurses can help patients control diabetes by introducing them to effective strategies that prevent bad outcomes.
Type 2 diabetes is characterized by high blood glucose levels, often known as hyperglycemia. The illness causes lifetime alterations in the sufferer. Type 2 diabetes is more likely to develop in people over 40, overweight, or with close relatives with the disease. Type 2 diabetes causes strokes, nerve damage, blindness, renal difficulties, foot sores and infections, and amputation (NHS, 2017). Thus, diabetes patients are more prone to developing diabetes type 2 complications. According to Wound Source (2019), diabetic foot ulcers affect 25% of people. Painful woods on foot diagnose foot ulcers. Severe wounds may necessitate amputation. This information is vital in identifying the patient's needs and devising an effective treatment plan.
Patient Description
The paper will not use their actual name to safeguard the patient's privacy. The policies required patient consent before starting the assignment (NMC, 2019). Personal data will not be utilized for undesired purposes. PatientXXX, a retired 70-year-old with Type 2 diabetes, will be the patient's name. Aside from diabetes, he was recently hospitalized for a wound on his right leg. PatientXXX has had abnormal blood sugar levels for the past six months with an Hba1c of 8.5. His memory lapses have hampered his ability to take medication for over a year. His BMI is 30.0, which is considered obese. PatientXXX is obese due to improper eating habits. Diabetes patients must eat healthily. Diabetics, for example, should not consume excessive sugar, fat, or salt (NHS, 2017). Patients with diabetes should avoid foods like chips, cheese, ice cream, and cakes since they are high in fat and sugar. Also, his injured foot prevents him from exercising, causing his blood sugar levels to rise. Exercise helps people with type 2 diabetes maintain lower blood sugar levels, according to NHS (2017). PatientXXX's wound prevents him from exercising and losing weight. He has trouble breathing when he walks short distances due to his exercise weight.
Overview of Nursing Theories
The Roper-Logan-Tierney (RLT) model provides theoretical principles relating to the patients' daily activities. Roy's Adaptation Model was developed to help patients adjust to their environment. RLT model is routinely used in the public health of the United Kingdom to assess patient care. It is identified as a checklist used to determine how an illness has affected the patient's life (Petiprin, 2016). It is used to look at a patient's daily activity to help nurses determine an effective way to help patients develop independence and improve their quality of life. The theory is characterized by a need to create meaning in life through interventions that support independence among patients. Sometimes patients may have difficulties getting through daily activities independently (Appendix 1). The goal of the daily activity assessment is to create support strategies for activities that patients cannot undertake alone (Petiprin, 2016). Therefore, nurses use this model to understand what a patient lacks in achieving independence. After that, they will develop a plan to improve the patient's abilities to attain freedom through nursing knowledge and skills. In Roper-Logan-Tierney's Model, patient assessment is categorized from complete dependence to complete independence.
By determining the level of support for the patient, nurses can develop appropriate interventions to improve the patient's life. When the patient is entirely dependent, the nurse has to identify ways to provide support to the patient physically and mentally to offset the dependency. Also, when the patient is entirely independent, the nurse has to evaluate his condition to determine the existence of any future dependence to create ways to prevent to improve his independence. Hence, nurses generate a checklist of living activities listed by Roper-Logan-Tierney's Model in the assessment. Rope urges that the activities should be treated as a cognitive approach and not as a list of checkboxes (Petiprin, 2016). The model requires nurses to pay attention to the assessment and focus on patient care organization. Evaluation is critical at the beginning of the nursing process to determine dependence or independence to create an effective evaluation plan. The evaluation plan helps the nurse determine any positive or negative changes in the patient, thus developing an efficient course of action that would boost the patient's independence.
The model lists ten activities of living those nurses should evaluate to determine a patient's dependence or independence. Roy's Adaptation model of nursing views patients as interconnected systems based on their biological, psychological, and social components. The model's creation was influenced by the concept of family, religion, background, and clinical experience (Petiprin, 2016). Roy states that adaptation is a positive response to environmental changes. Roy's Adaptation model consists of four elements; the patient, the nursing care they receive, their health status, and their surrounding. Practical nursing care requires proper integration between the four components. A patient's system structure (bio-psycho-social) constantly engages with environmental changes. The nurse's responsibility is to promote the patient's ability to adapt to ecological shifts by creating balance in their system structure. Nurses have to assess the patient's behaviour to determine their stimuli according to changes. In most cases of nursing practice, this model is commonly used for mental health patients.
Activities of living model
`The activities of the living model originated from the Roper-Logan-Tierney (RLT) theory to guide nurses through a process of promoting patient independence. Problems associated with the normal function of the human body are known to cause a decline in the well-being of an individual (Giebel & Sutcliffe, 2018). Many patients with chronic diseases have problems with independently undertaking activities of daily living. Hence the model was created to help the patient regain independence in their life. Giebel and Sutcliffe (2018) illustrate that patients with dementia have poor performance in living activities. The RLT model was developed to create a solution for patients with high dependency levels (Appendix 1). The model illustrates ten activities of living that are essential to independence. Nurses need to determine the role of “biological, psychological, sociocultural, environment, and politico-economic” factors in living activities (Petiprin, 2016). Nurses develop a holistic model by understanding the relationship between environmental factors and living activities. The holistic model is a nursing concept that focuses on healing the “whole person.” The “biological, psychological, sociocultural, environmental, and politico-economic” factors are crucial in the patient’s dependence or independence (Petiprin, 2016). Hence, when nurses determine the interrelation between the elements and activities of living, they understand the patient’s anatomy. If the factors are not considered, the assessment will not provide efficient information about the patient’s dependence. A wrong assessment results in an ineffective intervention that will not help the patient attain independence. Roper asks nurses to avoid presumption about patients, and they should communicate with patients effectively to understand their activities of living and preferences. Regardless of the area of treatment, nurses using this model should incorporate the five factors with the activities of daily living. Therefore, I chose the RLT model because it is relevant to my patient’s health issues and related to physiological problems like those of my patient.
