Question 1
According to Choi (2010),adult respiratory distress syndrome (ARDS) is a condition in which there is lack of enough oxygen supply to the lungs, which resultsin the life-threatening situations. Within the ARDS, there is a development of fluid in the alveoli, which prevents the exchange of oxygen to the blood. As indicated by Chiumello (2017), ARDS can lead to a complexity of different infections in the body, which in turn can result in the death of a person, dependingon the seriousness of the condition and the age of the person. ARDS has various associated clinical indicators, including reduction of oxygen level in the blood, increased rate of breathing, andreduction in the blood pressure
Question 2
John Smith experienced several conditionswhich are common risk factors associated with ARDS: pneumonia, increased transfusion of blood, infection of bacteria in the blood, and abuse of substances.
Question 3
As indicated by Russell and Walley (2010), the common V/Q mismatch associated with ARDS is the hypoxaemia, which occurs during the inequality of the pulmonary perfusion and alveolar ventilation. The V/Q mismatch is the most well-known cause of hypoxia in seriously sick patient and is caused by intrapulmonary pushing of blood, which occurs due to airspace filling.
Hypoxemia in ARDS is treated through mechanical ventilation. The clinical significance of static compliance is that it is an important measurement of the respiratory physiology (Choi, 2010). The decrease of static compliance is demonstrated in the patient’s case, where the value decreasesas lung volume risesdue to constraints of the non-flexible components of the lung wall system. According to Chiumello (2017), the decrease in static compliance reflects the consistence of the lung that is taking an interest in exchange of gas and not the fallen of alveoli.
Question 4
Russell and Walley (2010) state that the interpretation of the acid-base balance of this patient is that the respiratory alkalosis is a disturbance in acid-base balance because of the carbon dioxide (PaCO2) pressure. The reduction in PaCO2 rises the bicarbonate concentration ratio to PaCO2, whichconsequently increases the level of the pH, thus leading to respiratory alkalosis (Russell and Walley, 2010). The cause of imbalances is caused by the reduction in the PaCO2, which occurs when a solid respiratory stimulus makes the respiratory framework to remove several PaCO2 delivered metabolically in the tissues
Question 5
Russell and Walley (2010) indicate the following effect of PEEP on pulmonary and cardiovascular status of the patient: in the normal breathing, the negative pressure inspirationhelps in the venous return, alleviates pressure on the pulmonary vessels, and improves the blood flow. Moreover, the pressure of intrathoracic increases due to the positive pressure ventilation, causing a reduction in venous return and pulmonary blood flow together with the right ventricular output.
Question 6
According to Choi (2010), the situation for ICU requires interventions of nursing to solidify the professional character based in view of a reasonable perspective of the phenomena that are considered in the field of nursing. Therefore, the primary nursing diagnosis for this patient upon admission to ICU is through a cross mapping of terms contained in nursing records with the NANDA-I taxonomy which identify nursing diagnoses in the ICU patient (Choi, 2010). The Nanda International Nursing Diagnoses (NANDA-I) taxonomy play a significant role as they portray one of the phenomena of interest for the expert practice in a standardized way.
Question 7
One of the nursing interventions is to implement measures to enhance respi...
