There is no doubt that prevention is better than cure, yet very little is known about the delivery of health promotion interventions in our national medical programs. Disease prevention and health promotion programs aim at upholding individual health. Health awareness program focuses on empowering and engaging people and communities to practice healthy activities and initiate changes the minimizes the risk of developing chronic illness and other diseases. World Health Organization has defined health promotion as the method of empowering individuals and communities to heighten control over and to improve their health. Disease prevention and health awareness programs frequently focus on social determinates of health which impact adaptable risk activities. Examples of social determinates of health include social, political, cultural and economic environments in which individuals are born, grow and live that affect their health condition. Adaptable risk activities include poor eating lifestyles, use of tobacco and lack of physical activity, which leads to the development of chronical illness CITATION Fro14 \l 1033 (Fromme, 2014).
Several theories and models have been developed that support the practice of disease prevention and health awareness. These models and theories are used in program planning to describe and recognize health behavior and to direct in identification, improvement, and application of the interventions. In the process of selecting a model or theory to monitor disease prevention and health awareness programs, it is crucial to deliberate on many factors such as precise health issues, type of demography, and the nature in which the program is being applied. Models and theories applied in disease prevention and health awareness programs include; health belief model, social cognitive theory, trans-theoretical model, social-ecological model and theory of reasoned action. Disease prevention and health awareness programs are usually derived from one or more of the above models and theories.
