Population health management
Population Health Management offers firm, and wide-ranging strategies for refining the structures and policies that affect the excellence, access, and results of health care. The goal is to advance the health of the whole population, (McAlearney, 2003). However, the entire population’s health is tougher to quantify due to the outsized health care industry that accounts for approximately 14 percent of the country’s Gross Domestic Product (GDP), (Kindig, 1998).
According to Chandra (2015), "Crossing the Chasm for Population Health Management – from Pilot to Enterprise Scale”, despite the fact that the exact procedures for measuring the population health and their outcomes are vague, health programs face challenges. These include cost, inadequate resources, lack of staff and the incapability to harmonize their efforts among the participants. Hence, for a program’s success, people with the related goals support the business process and the outcomes of the population health program by consolidating their efforts.
However, McAlearney (2003) maintains that challenges are evident in spite of organizations and delivery structures developing the ability to report comprehensive measurement concerns. This is based on the perspective that the Health-Adjusted Life Expectancy (HALE) is significant in measuring population health results.
Contextually, Chandra (2015) proposes a strategy that may ultimately remedy the outcomes that may aid any population health program to become successful like the Montefiore Medical Centre. Chandra (2015), suggests;

This is echoed the in healthcare system under the population health programs. Illness, incapacity, cataclysmic, daily life, demand management are examples of the scopes that different population health management programs are targeted, (McAlearney 2003). These strategies are individually tailored to meet certain groups in trying to influence the outcomes of the populace. Chandra (2015) maintains that for the realization of specific outcomes of a particular program, it is imperative to remain competitive in a particular target population.
Propose the structure for individuals, procedures and expertise.
It is crucial that any program does not rely on one technology to use to reach their goal but instead to take on all facets that would make the program wholly satisfactory. Therefore, it is important to analyze who the project requires and their expertise in the particular field; policies and measures used, risk and information systems, (McAlearney 2003). Focusing on operations is essential in population health programs for accessibility and encountering its health or economic objectives.
Implementation and adaptation.
Population health management program remain vital in the provision of quality and accessible health. Hence, once all the parameters are considered and sourced, the implementation should be immediate. This is due to the health uncertainty of the populace, and new strategies, and ways of dealing with health issues are emerging, (Chandra 2015). Outfits used in decision making and keeping track of the programâ...
