Healthcare system errors, including medication errors, cause both economic and care outcome implications. Primarily, the high rate of medication errors in healthcare systems necessitates a system-wide change with an outlook to change care outcomes for the better. Medication errors can occur due to various processes and aspects that relate to care provision. Medication errors occur due aspects relating practitioner qualifications, distractions, and due to environmental factors. Therefore, errors in the healthcare systems can be caused by medication events, surgical errors, laboratory errors and healthcare-associated infections. This way, there is a need to develop and implement system-wide changes that will help to reduce the rate of medication errors.
