Several factors might have interfered with the pharmacokinetic and pharmacodynamics processes in Patient MW who was diagnosed with Generalized Anxiety Disorder, including age, co-morbidity, and other drugs or treatments. Research shows that patients above the age of 65 years, like MW, have greater levels of CYP2C19 genotype, which affect the metabolization of antidepressants (Strawn et al., 2018). Also, MW suffers from other illnesses such as Hypertension, alcohol abuse, obesity, and vitamin D deficiency, all of which affect the rate of absorption, efficacy, and excretion of the drugs (Health Quality Ontario, 2017).Finally, MW is taking other drugs and supplements, such as Amlodipine, Atorvastatin, Folic acid, and Vitamin D, which might influence the efficacy of the pharmacokinetic and pharmacodynamics process.
Notably, there are different treatment options for patients like MW. Evidence suggests that the SSRIs (Selective serotonin reuptake inhibitors) and SNRIs (Selective serotonin-norepinephrine reuptake inhibitors) qualify as the first line of treatment due to their high levels of tolerability and efficacy (Slee et al., 2019).The second line includes treatments such as benzodiazepines and buspirone, pregabalin, and Gas, which were prescribed for patient MW. Nevertheless, such patients also need psychotherapy treatment as research shows that such treatments enhance antidepressant response. Psychotherapy is also crucial in this case due to non-compliant and drug abuse behaviors that may inhibit the effectiveness of pharmacology. According to “multimodal approaches (e.g., psychotherapy and pharmacotherapy) may differentially target specific symptoms (e.g., cognitive vs. somatic), and the addition of psychotherapy increases treatment adherence and decreases reported side effects of pharmacotherapy" (Strawn et al., 2018).
