Clinical Manifestations.
Mr. M. is a patient who is 70 years old. Mr. M claims he has no allergies. Mar M has a shaky gait and has difficulty ambulating. Mr. M suffers memory gaps because he has trouble remembering the name of his relatives, the number of his bedroom, or repeating what he has spoken. Mr. M exhibits both fear and aggression. Mr. M is unable to bathe, clothing, or eat himself without assistance. Mr. M is pale and exhausted owing to sleep deprivation, and he takes Zolpidem to treat his insomnia. Mr. M has an average temperature, based on the objective data gathered from the lab tests, and thus no indications of fever. Mr. M displays no signs of pain and claims that he is not in any pain. After assessing Mr. M's BMI, he discovered that he is overweight. Height: 69.5 inches; weight: 87 kg, or a BMI of 28.4. Mr. M's urinalysis revealed a significant quantity of leukocytes and was hazy.
Primary and Secondary Clinical Diagnosis.
The patient has a C BP of 123/78, a heart rate of 93, and a respiratory rate of 22 percent, all of which indicate prehypertension CITATION deM21 \l 1033 (de Menezes, 2021). In the case that systolic and diastolic pressures fall into separate ranges, the worse size is utilized. Furthermore, those with high blood pressure have a higher risk of having a heart attack than normal blood pressure. Attributed to the prevalence of leukocytes in Mr. M's urine, the second primary diagnosis will be to examine UTI. Urine with an unusually high number of leukocytes may indicate a urinary tract infection or edema. Mr. M. needs a diagnosis since he is taking medicines such as  Lisinopril, both of which have documented renal adverse effects. Detection of urinary tract infection, therefore, is essential.
