Reflective account: ABCDE ASSESSMENT OF BRENDA (SBAR Report included)What was the nature of the CPD activity and/or practice-related feedback and/or event or experience in your practice?
A home visit to Brenda who had just returned from a four-day hospitalisation due to her deteriorating COPD. An ABCDE assessment was done, and these were the findings;
Airway was not sufficiently clear evidenced by the patient coughing out sputum, which indicate airway blockage. Even though, the patient could talk clearly in full sentences which show minimal airway disturbance.
Breathing: No abnormal breathing patterns were identified. The respiratory rate was 18 and 16 breaths per minute on two different occasions, and oxygen concentration in the blood was 98% and 96% respectively. These meet the target saturations, which were set at 94-98%. No signs of respiratory distress were noted. These indicate standard breathing patterns.
Circulation- On two different occasions, the pulse rate was 88 then 78 beats per minute, blood pressure readings were 140/90 mmHg then 135/80 mmHg. Final reading of the general body temperature 36.5o C. All
