NUTRITIONAL CARE PLAN.
Tariq Jones requires a nutrition care plan after he was diagnosed with the Crohn’s disease. A proper nutritional care plan is important during and after treatment, hence if not followed may results to some dire consequences. Nutritional risks that Tariq may fall into if a well nursing plan is not followed may be; deficiency of micronutrients and micronutrients, weight loss, dehydration and lastly malnutrition.
Severe diarrhea as experienced by Tariq may cause dehydration, meaning he may have less of fluids, nutrients, and necessary electrolytes such as sodium, potassium, magnesium, phosphorus, and zinc. It may also lead to weight loss. The rectal bleeding that he experienced from intestinal ulcers may cause deficiency of iron. In addition, abdominal pains he went through may bring about lack of appetite and make him unable to add nutrients or energy to his body. Hence might end up being malnutrition as stated by Nguyen, G. C. (2008).
Micronutrient and micronutrient deficiency is also a nutritional risk for Tariq. Micronutrients refer to trace minerals and vitamins required in small quantities in the body. However lack of them may cause overt clinical syndromes like anemia and night blindness. Macronutrients include fats carbohydrates, calcium, proteins which are found in large amounts, which are very fundamental in one’s body as explained by Dunnellon, C. F. (2013).
INTERVENTIONS AND EXPECTED OUT COMES
An enteral nutrition will be administered in the elemental type of formulation. A change of diet will be required. The expected outcome may be an improve inflammatory and nutritional indice. In addition a cessation of the persistent symptoms is expected as explained by Di Caro(2019)

THE APPROPRIATE DIET
1 Refined grains pass quickly through the digestive tract and are less fermentable, for example, white rice and pasta which are rich in iron and iodine
2 Oatmeal contains a soluble fiber called beta-glucan. It can help ease diarrhea by absorbing water in the intestines, forming a gel, slowing digestion, and adding bulk to stool.
3 Low-fiber fruits are easy on the digestive system, and they can help control diarrhea. For example, bananas and watermelon. However during flare up moments, it advisable to serve them in small quantities.
4 Peeled and cooked vegetables. Cooking reduces fiber contents therefore eases digestion. However too much oil is not advisable hence steaming and boiling is advisable.
This means that Tariq will a dietary modification of their regular diet. Enteral feeding is necessary since Tariq has had severe diarrhea and lack of appetite for about 5 days. The enteral feeding first out to be administered orally and if ineffective the tubing is considered. The elemental formulation is to be used on Tariq since he may have an impairment of the gastrointestinal tract as stated by Di Caro. (2019)
NUTRITONAL EDUCATION SESSIONS WITH FAMILY AND PATIENT
It is important that the patient needs to know the following:
* How much to take, when to take it and for how long
* Possible food and drug interactions.
* Expected action of the medication and possible side effects
* What to do if a dose is missed.
* Special directions for mixing and administering the medications
* Proper storage and expiration and disposal.
This way it will be easier for the patient to know what foods to take and in what frequency and the consequences of not taking the prescribed medication. Knowledge to the family is important in that it enables them to:
* What foods to avoid, especially to prevent food and drug interactions.
* Special diet instructions
* How to read food labels.
There are a few important stages during behavioral changes for patients undergoing nutritional care. One of the important stages is the trial stage, this where the patient has tried the behavior required but has faced difficulties, therefore reinforcement from people around him is required.
