The assessment was conducted on Hillington. The patient, Mr. Hillington, aged 76 has smoked a pack a day for 50 years now. Mr. Hillington insists that he has not used any kind of intoxicating substance. Plavix, Nexium, Librium, and Enbrel are the medications being taken by the patient at the moment. The patient's medical background includes a history of hyperlipidemia, heart disease, arthritis, and COPD.
During the assessment, I used three assessment tools, the Geriatric Depression Scale, Katz Activities of Daily Living, and the Lawton Instrumental Activities of Daily Living Scale. When assessing a client's functional state, several instruments grade them on their level of independence and their proficiency in doing ADLs. Using the Katz tool, a patient receives 1 point for performing an activity without help, whereas a client who receives a score of 0 requires complete care. Dressing, washing, toileting, transferring, continence care, and feeding are all mentioned. Clients may be classified as dependent (with a score of 0) or independent (with a score of 6). Depending on the score, you're either halfway dependent or fully independent.
Lawton's instrument evaluates more complex aspects of everyday life such preparing food, cleaning, using the phone, going shopping, doing laundry, getting about town, taking care of one's medications, and managing one's money. To eliminate discrimination based on gender, the scoring range for women is 0-8 and for men it is 0-5 on a scale from zero to five. From 0 (very low) through 5 (average) to 8 (very high).
