Medication errors remain a significant challenge in healthcare. There occur an average of 100,000 fatalities each year in the US arising from medical errors; with thousands of other patients undergoing adverse health complications (Gaffney, Hatcher, Milligan & Trickey, 2016). The US spends over 6.5 billion dollars every year on costs resulting from medication errors (Gorgich, Brafroshan, Ghoreishi & Yaghoobi, 2016). Reports show that 25% of all litigation claims touching on the medical field are as a result of medication errors (Aronson, 2009). A large number of cases are noted in time and corrected before the patient suffers serious health damage (Wittich, Burkle & Lanier, 2014). Other times errors are not identified or corrected in time and patients or their caregivers end up suing hospitals for professional negligence.
Medication errors are defined as preventable mistakes occurring in the administration of medication of a patient and include prescribing wrongful medicines or dosages; failure to prescribe required medicines at any stage of the treatment process or forgetting to give medication in time (Wittich, Burkle & Lanier, 2014). Medication errors also occur in the mode or route of administration, formulation dispensation or monitoring therapy (Aronson, 2009). Most medication errors are not life-threatening and cause little harm to the patient. That notwithstanding, they are unwarranted and unwanted. They heighten medical costs, prolong hospitalization and generally result in loss of confidence and trust in the healthcare organizations concerned (Gorgich, Brafroshan, Ghoreishi & Yaghoobi, 2016).
