The population health approach may be defined as a framework that aims to improve the health conditions of the entire population in a community. Such improvement also requires the reduction of health inequities existing in the said population. Policies incorporated in this approach also improve the physical and mental outcomes of patients. The best practices in this framework inform the best programs, health policies, and available resources to enhance the population’s health. A case study analysis on Montefiore Medical Center will provide more insight into how a population health approach works and the cultural competence of the center established through patient engagement, health promotion, disease prevention frameworks, financial incentives, and quality improvement processes.
Patient Engagement Activities and Communication Strategies
Translation and Interpretation Services
Montefiore Medical Center has implemented a patient-centered system tailored to the needs and preferences of the community. For instance, the center has adopted an effective interpreting service program that caters for the needs of the deaf, blind, and patients who experience challenges in communicating using the English language. In addition, the center has heavily invested resources to ensure communication becomes an effective tool that would promote its objectives. Montefiore has immunization programs where they offer vaccines for illnesses such as measles and chickenpox. Owing to the lack of a proper and efficient transport system, Montefiore Medical Center has invested its resources and procured shuttle services for its patients. As a result, patients can access transport without worry and attend their appointments with nurses or physicians. The health care center also conducts mobile clinics as they take health services closer to the people, which amounted to 25,000 visits in 2010 (Montefiore Medical Centre, 2021). The health care center has also provided on-site services to the homeless shelters around the Bronx community.
Gaps or Deficiencies
Montefiore has implemented many policies and procedures to ensure it becomes effective in its operations around the community. Its main drive is to ensure the betterment of health among the patients and overall community members. The health care center does not operate in a vacuum, and it experiences challenges in existent gaps and deficiencies. Nevertheless, these gaps do not condemn the center into a doomed status as the center can conduct activities to perfect its productivity. Some existing health care problems which have plagued the community include substance and drug abuse, which is on the rise. The center should consider addressing this issue as it, directly and indirectly, impacts the community’s health, which it is trying so hard to enhance.

Any health care center requires implementing effective policies aimed at promoting a good health status of the community. Over the years, Montefiore Medical Center has tailored its policies to provide better health care services to the community. In the wake of continued racial discrimination among the minority groups, Montefiore health care center has observed an opportunity to employ health care providers from different racial and ethnic backgrounds to take care of the patients. Its non-discrimination policy validates this position. This policy also ensures practitioners and care providers are hired based on merit and qualifications rather than ethnic background. The policy also shields ethnic minority patients who want access to quality health care services without prejudice.

Montefiore Center delivers comprehensive, integrated health care to its patients.
