Introduction
Health promotion practitioner involves practicing things to improve communities and individual health and to prevent illness. Health practitioner offers certain solutions to several health issues facing by individuals and communities such as smoking, lack of exercise and obesity through development of knowledge and skills, health services, health related public policy. Supportive environments and community action (Armstrong et al, 2017). Health care practitioners involve in several roles starting from prevention of health problem to promote the health of people suffering from terminal illnesses. One of the important roles of health promotion practitioners is they involvement in advanced care planning (Lobo, Petrich, & Burns, 2014). Advanced care planning permits health promotion practitioners to respect and understand health care preferences of persons in the future, for the time when patients become seriously ill and they will be unable to communicate or coordinate for themselves. In advanced care planning, health promotion practitioners identify and assess needs of patients through effective communication with patients, their family members and also to other health care professionals that are directly or indirectly involved in patients care (Li, Carter, & Rychetnik, 2015). While identifying needs of the patients, it is very important to understand the culture and relationship of patients with other stakeholders in order to plan by considering all important factors.
