Culturally appropriate Nursing Care for LGBTQ+ populations Slide 1 Culturally appropriate Nursing Care for LGBTQ+ populations Slide 2 Introduction Slide Points 1 LGBTQ+- Lesbian, Gay, Bisexual, Transgender, Queer, and others people recognized beyond heterosexual norms. 2 This is a diverse community with different gender identifications like transgender, gender-fluid, non-binary and have various sexual orientations like bisexual, pansexual and asexual. 3 In the U.S. 7.6% of people are identified under this category means 1 in every 5 GenZ adults are LGBTQ+ (Jones, 2024). 4 These individuals are disadvantaged as they face different healthcare issues like increased mental health issues, higher substance abuse rates, culturally incompetent care and also encounter bias. Figure SEQ Figure \* ARABIC 1 LGBTQ+ percentage (Buchholz, 2023) Slide notes It is significant to understand this diverse population as they develop a sense of mistrust towards the medical filed so it is important to deliver patient centered and inclusive care to them. Most effective care would be culturally competent care that needs to be provided to them which will help lessen the discrimination they face. Therefore, Nurses need to use gender affirming language, respect their identities and actively listen to them. Slide 3 Characteristics and health needs Slide points 1 Mental health of LGBTQ+ is 15. To 2.5 times more drastic as they suffer from anxiety and depression in comparison with heterosexual people (Gmelin et al., 2022). 2 Substance abuse is also common among this population which is 25.1% in comparison with 18.8% among heterosexual populations (Garfield et al., 2023). 3 Apart from these, the ratio of chronic disease among LGBTQ+ population is also high (67% of gay and bisexual diagnosed with HIV) in comparison with heterosexual counterparts (Centers for Disease Control and Prevention, 2024). This eventually enhance the feelings of discrimination and stigma in them which subsequently impact their reproductive health and access to care facilities. Further, with proper care they might het a chance to involve within the community. Addressing mental health issues will also help reduce the suicide rates and substance abuse rates. This will not only enhance trust but also encourage them to get medical help for positive health outcomes. Slide 4 Cultural values and beliefs Slide points 1 To upkeep the cultural values and beliefs of LGBTQ+ community it is essential to use correct pronouns by the care professionals. Respecting their identity will foster positive mental health and overcome the care neglect in them. Statistically it has been proven that with proper gender-affirming interventions received by transgenders, these has been 60% less cases of depression and 73% less instances of self-harm cases (Adler, 2022). 2 Additionally, disclosure barriers need to be reduced as these community individuals unable to get proper care and treatment due to increased stigma. 3 LGBTQ+ population also suffer from healthcare disparities due to negative beliefs about them, lack of knowledge among providers regarding them, societal reasons like homophobia and transphobia (Bass & Nagy, 2023). One major advantage would be enhancing trust, that eventually improve healthcare disparities. Further, nurses must understand non-traditional family structures for providing inclusive care, that will lower the issues of social disparities like stigma. Slide 5 Healthcare disparities Slide Points 1 These community also suffer from healthcare disparities due to lack of friendly providers, economic barriers and systemic discrimination. 2 According to the statistics, in 2017-18, among 76% participating schools, nearly half reported 3 or less learning activities about LGBTQ+ health, subsequently aiming towards uninformed care (Streed et al., 2019). 3 These populations also suffered from lack of insurance coverage for gender affirming hormones as 17% of them do not possess any kind of health insurance in comparison to 12% of non-LGBTQ+ population (Human Rights Campaign, 2019). This links with lowering overall odds of hormone uses and enhancing nonprescribed hormones. Slide notes These disparities in getting healthcare facilities among LGBTQ+ population shows that these are systemic barriers, financial obstacles and inadequate or improper research about them. With increased disparities such population further develop more mental ailments, chronic conditions thus need more focused care. Moreover, no insurance coverage also reflects upon unmonitored or unsafe medical practices towards these vulnerable populations which leads to increased adverse healthcare conditions. if they are provided with proper access to hormones then it might lower their economic burden as well. Slide 6 Strategies for providing culturally competent care Slide points 1 AI powered heath training for LGBTQ+- this strategy would include the measures to integrate AI simulation in the operations which is used by nursing practices to enhance communication and enable them to make clinical decision-making. 2 Patient navigation services- For LGBTQ+ populations it is necessary to integrate focused healthcare navigators to help them align referrals, insurance policies and culturally knowledgeable providers (HRC, 2020). 3 Gender affirming virtual care- this strategy must also be implemented to provide care to the LGBTQ+ populations in the form of telehealth approaches which aids in providing inclusive, person centric remote consultations. This will help in increasing access to healthcare services to LGBTQ+ patients. Slide notes Certain examples like installation of VR and AI in the social work like analyzed in NYU Virtual Reality can prove essential. This can be helpful for the new generation students to enhance their real-world experiences and interact with patients effectively (NYU Web Communications, 2024). For gender affirming care, Equinox is one significant example which is peer led for trans and gender diver population under which care regarding sexual health, hormone management and counseling will be provided (Prism Health, 2024). Slide 7 Case study 1 Discrimination Slide points In 2018, Evan Minton, a transgender man was denied suitable care of hysterectomy at Dignity Health’s Mercy San Juan Medical Center in California because of being transgender, illegal discrimination done against him by the providers (ACLU, 2021). This denial from the care providers instigated a law suit which highlighted discriminatory policies. According to the statistics, 56% of transgenders stated that they have faced discrimination in medical facilities which aimed to reduce the healthcare use and avoidance (Zalabak & Gantioque, 2024). Slide notes In this case advocacy efforts were prompted to improve the non-discrimination laws for such communities in healthcare. Moreover, this case also revealed increased discrimination which these vulnerable population faces in the US thus giving emphasis on policy changes and health reforms. Slide 8 Case study 2 Gender-affirming care barriers Slide points The case study involves Alice Litman, who was a transgender woman from Brighton, England. Alice was compelled to took her life when she was 20 years of age due to increased wait times of 1,023 days for her first appointment with gender identity development services (GIDS) (Brooks, 2023). This shows a NHS drawback regarding increasing wait times for transgender patients. Based on statistics of William Institutes, it was stated that 81% of transgender in the US thought of suicide, and among them 42% have attempted it. The rationale behind it aligns with the psychological distress among the population (Dowd, 2023). Slide notes This case study reflects upon increased anxiety, depression and suicidal ideation issues which LGBTQ+ populations have to face while getting healthcare. In social terms, these populations also have to face alienation in the community, discrimination in office, stressed relations which lowers their quality of life that are because of lengthy gender dysphoria. Conclusion In the end, it can be analyzed that LGBTQ+ population across the globe face major healthcare disparities because of discrimination in the society, lack of accessible services and care and other systemic barriers.
