The plan of care should be based on history, physical assessment findings, laboratory data, and diagnostic test results. The program of care should be reassessed regularly. Documentation for care provided should be clear and complete when transferred to other providers. The nurse should provide timely education to the patient and family regarding restrictions, requirements and follow-up. All post-delivery assessments (after 24 hours) need to be documented by using the Postpartum Checklist or a similar resource (if a written checklist is not available).
The primary goals for a postpartum plan of care are to assess the emotional recovery from childbirth and establish a plan for patient and family education. Common emotional complications associated with childbirth include depression, anxiety, post-partum blues, fatigue, lack of confidence in mothering skills, and sexual dysfunction.
