Nursing care plan
Formulation of a nursing plan of care would be informed by the need to address the symptoms and signs of schizophrenia in the patient. This plan of care would enable the patient to improve her condition that will allow for a full recovery in due course. The first step toward this intervention would be to recognise the presence of schizophrenia. This would involve noting the characteristic symptoms and signs of the condition, such as speech abnormality, thoughts distortion, and poor social interactions. The plan of care would also enable the establishment of trust and rapportCITATION Pin17 \p 90 \l 2057 (Pinho, et al., 2017, p. 90). In this regard, it will be important to take care of the patient and expecting the patient will need to be understood in all aspects. It will be important to make a close relationship with the patient and establish trust. The plan of care will also be informed by the need to maximize the level of functioning of the patient. In this regard, it will be fundamental to assess the ability of the patient to carry out Activities of Daily Living (ADLs). To facilitate full recovery of the patient, it will be imperative to have a positive symptom inherent in the patient. This will involve assessing if the patient has command hallucinations, presence of fragmented, poor organised, well-organized or systematized system of belief that are not supported by reality.
