P. D. is brought to the emergency department complaining of a sore shoulder. He states that he was forced off the road while riding his motorcycle and sideswiped a telephone pole. He did not lose consciousness. A urine specimen reveals gross hematuria. P. D. also states that a tape player in the right pocket of a leather jacket is shattered.
A family of two adults, mother age 33 years and father age 29 years, and two children, ages 6 months and 3 years come to the local health clinic. The adults state that they have recently relocated to this area and are interested in finding a health care provider. When asked, the mother says there are no known health problems. During the interview with the parents, you note the 3-year-old playing with a puzzle.

Case study 1: “p. D. Is brought to the emergency department complaining of a sore shoulder. He states that he was forced off the road while riding his motorcycle and sideswiped a telephone pole. He did not lose consciousness. A urine specimen reveals gross hematuria. P. D. Also states that a tape player in the right pocket of a leather jacket is shattered”.
Subjective data is measurement and can be easily observed (Mosaly, Mazur & Marks,2016). Thus, in this case, the objective data is wound appearance, urinal condition, blood pressure, and heart rate. On the other hand, the objective is defined as what is being described by the patient; thus, it includes; sore shoulder, being forced off the road while riding his motorcycle, sideswiped a telephone pole, and a state of consciousness. The nursing diagnosis in the case study includes; risk of infection, kidney injury, or condition which might have resulted in hematuria. The wellness, health promotion, and disease prevention activities that apply for this case study are education, which aims to raise awareness of various strategies to help handle such kinds of accidents whenever they occur. Also, there is the policy, systems, and environment focused on enabling a healthy physical environment and the use of protective clothing while on a motorcycle. The developmental considerations applied while gathering data in this case study are comparing the already obtained data and the developmental parameters of the p.d. This will include analyzing the data; sore shoulder, urine specimen, state of consciousness (from heart bit rate and blood pressure) against the parameters of psychosocial status, weight, height, hereditary conditions, and developmental delay. Socioeconomically, the consideration entails whether p.d will be able to afford the treatment regarding his background. On the other hand, cross-cultural relations entail things such as language barrier, treatment expectations, and vivid descriptions of the causes of the hematuria.
Case study 2: A family of two adults, mother age 33 years and father age 29 years, and two children, ages six months and three years, come to the local health clinic. The adults state that they have recently relocated to this area and are interested in finding a health care provider. When asked, the mother says there are no known health problems. During the interview with the parents, you note the 3-year-old playing with a puzzle.
In this case, the objective data is the age of the mother, father, and two children. This can be obtained from their birth identify certificate or health records, thus qualifying as subjective data. On the other hand, the objective is defined as what is being described by the patient. Therefore it includes; relocation of the family to the new place and the family history on health problems (Birkhäuer et al., 2017). Furthermore, the nursing diagnosis in the case study includes; lack of knowledge, as the mother noted the absence of health problems, and at the same time is in search of a health care provider.
