Chief Complaint
A.C., is a 61-year old male with complaints of shortness of breath.
History of Present Illness
A.C. was seen in the emergency room 1 week ago for an acute onset of mid-sternal chest pain. The event was preceded with complaints of fatigue and increasing dyspnea for 3 months, for which he did not seek care. He was evaluated by cardiology and underwent a successful and uneventful angioplasty prior to discharge. Despite the intervention, the shortness of breath has not improved. Since starting cardiac rehabilitation, he feels that his breathlessness is worse. The cardiologist has requested that you, his primary care provider, evaluate him for further work-up. Prior to today, his last visit with your practice was 3 years ago when he was seen for acute bronchitis and smoking cessation counseling.

Pathophysiology & Clinical Findings of the Disease
1 Are the spirometry results consistent with obstructive or restrictive pulmonary disease? What is the most likely pulmonary diagnosis for this patient?
The Spirometry results were consistent with the obstructive and restrictive pulmonary disease. From the measurements, the FEV (1) was 64% which was way below the normal range of 80% - 120%. Similarly, the measurements also showed that the absolute FEV1/FVC ratio was 69% which was higher than the recommended normal ratio (Barreiro & Perillo, 2004). The ratio should be within 5%. From the clinical guideline practice (GOLD report, 2022; Bhatt et al., 2014), the FEV1 and FEV1/FVC ratio indicated that the patient had a restrictive ventilator impairment. From the justification, Spirometry results were evidently consistent with the obstructive and restrictive pulmonary disease. Considering the historical background of the patient, medical history and the condition the patient presents with, the patient is most likely to be diagnosed with Chronic obstructive pulmonary disease(COPD). COPD is an inflammatory lung disease with symptoms such as breathing difficulty, tightness of the chest, lack of energy, weight loss, swelling of the legs, coughs and wheezing. Most of which were present in the patients diagnostic process. Additionally, it is caused tobacco smoking and long term chronic diseases.
