This paper focuses on people with diabetes as a group that is at increased risk of readmission. Hospital readmission remains of the key parameter of measuring quality health care as well as the target area for cost reduction. There is a growing interest regarding the need to reduce preventable hospital readmissions .it is becoming a priority for payers, providers, and policymakers seeking to improve health care and lower health expenditure. With growing burden of diabetes in U.S, significant population of affected population bore the brunt of readmission due to worsening health outcomes. Diabetes is seventh leading cause of death in the U.S according to 2019 health data (American Diabetes Association,2018). Beside morbidities and mortalities, diabetes accounts for more than $327 billion based in 2017 data for both direct medical costs and productivity (American Diabetes Association,2018). With suboptimal management of diabetes, there is increased risk of readmission of affected patients. As a result, it is vital elaborate the relationship between population living with diabetes and its increased risk of readmission.
