1. Patient #2: Today you have two patients on your schedule for the HPV vaccine. One is a 27-year-old female. In taking her health history, you find that she is sexually active and that she had a splenectomy when she was 17. The second patient is an eleven-year-old female whose mother is concerned and wants to discuss the vaccine further before consenting. She feels that by giving the child the vaccine she is giving her permission to be sexually active.

In the case of the 27-year-old female with a history of splenectomy, I will recommend HPV4 (Gardasil) immunization. HPV4 is effective in women of up to 45 years (Mejilla et al., 2017). HPV4 has been proven to be safe in women with a wide range of health challenges such as chronic end-stage renal disease, liver and kidney failure. Therefore, HPV4 vaccination will help protect the patient from HPV types 6, 11, 16, and 18 (Mejilla et al., 2017). The vaccine will also prevent cervical and vaginal cancer; cervical adenocarcinoma in situ; genital warts; and vulvar cancer (Mejilla et al., 2017). In the case of the 11-year-old female, I will recommend Gardasil-9, also known as 9vHPV. 9vHPV was approved by Food and Drug Administration (FDA) and will be effective in preventing 6, 11, 16, 18, 31, 33, 45, 52, and 58 HPV types f the 11-year-old, I will recommend that she gets the second dose in the next 6-12 months. Since this is her first dose, it is recommended that she receives a 2-dose schedule. I will also ensure I educate the mother on the benefits of HPV vaccina
