“Continuity of care is among the vital processes in healthcare. It involves a team of individual providing care and the patient and aims at ensuring that the patient is satisfied with the services he/she receives” (Richard Dee, RN). Care management is regarded as the activities carried out by care providers to enhance the care of a patient. Case management, on the other hand, refers to the technique that is applied in care management whereby population managers monitor the outcomes, health statuses and resources for a particular targeted group of patients (Cary, 2016). Advocacy can be defined as the use of individual position to project, speak out or support the interests as well as the rights of another individual. In nursing, a nurse is ought to fight for the rights of his or her clients. In my opinion, advocacy and case management works hand in hand.
