Community Resources to Address Hypertension


Hypertension can be tackled using healthcare technology, the coordination of care, and community resources which provide channels for the prevention, treatment, and management of the medical condition and other related problems. Over the recent years, advancements in technology have been essential in efficiently managing patients and their health to enhance the overall quality of care. According to Omboni et al. (2018), telemedicine is applied in managing hypertension through blood pressure telemonitoring that enables the transmission of critical patient information, including blood pressure information, remotely. There is a significant increase in antihypertension medications in patients using blood pressure telemonitoring and remote counseling. Since the adoption of modern practices promotes better self-management of hypertension, there is a need to move away from traditional health methods in the management of the medical condition to control the disease and reduce the risk of related medical risks, including stroke, kidney failure, and cardiovascular diseases (Crilly & Kayyali, 2020). In addition, cost-effective control of hypertension is vital to reduce the hypertension burden on patients, the health system, and the general population. It is the responsibility of the baccalaureate-prepared nurse to promote patient health and enhance patient safety in their practice through maximizing the use of technology, providing holistic coordination of patient care throughout care delivery, and leveraging community resource services to attain positive outcomes and improve organizational effectiveness.
Impact of Healthcare Technology on a Patient, Family, or Population Problem
In the light of digital health information technology, new approaches are crucial to dramatically changing the quality of detection and management of hypertension. Such technology has significantly increased the number of measurements in various situations while allowing for the accurate detection of hypertension. In addition, E-health has increased the level of reach through the provision of remote patient care. Omboni et al. (2020) study that patients and health practitioners connect even outside the hospital setting for patient-centered care that ensures continuous patient monitoring and access to medical information, which improves their health behavior and increases their acceptance of the necessary lifestyle modifications and treatment options relevant to managing hypertension. Furthermore, the advantages of particular digital health information technology, including Electronic Health Records (EHR), have improved information sharing of patients’ medical histories, thus enhancing patient care and facilitating professional behavior. Lv et al. (2017) highlight that since EHRs information is instantly available wherever, it allows access to evidence-based tools that can be adopted for enhanced decision-making about a patient’s care. The complete information from such medical technology is further essential to reducing safety risks. The rapid growth in technology has increased the adoption of clinical decisions, teleconsultation, wearables, online prescribing, telehealth, and grid technologies in a healthcare setting for improved care coordination.
