Introduction
Mrs. Jane Smith was brought to the Emergency room complaining of dyspnea. She had previously been diagnosed with a heart failure (HF) class III, two years ago and her recent ejection fraction (EF) was 25 percent. She has no known drug allergy and is on medication, sodium and fluid restriction. She is currently in the acute medical ward and due to be discharged when she stabilizes. Mrs. Smith lives with her husband, who has been assisting her with her ADLs, as she has become less physically active. Her current status is edema on both lower legs and shortness of breath. Palliative home based care will be used to manage her condition once she is discharged.
Palliative Care Approach
Mrs. Smith is due to be released once she gets better and her treatment will continue at home. Palliative care at home will ensure that she is more comfortable and her family will be there to assist in her recovery. The co-create theory of nursing care is care that goes beyond an individual-nurse and the patient. It includes the family, the medical team, volunteers, community health workers and the local clinic or hospital (World Health Organization, 2016). The goal of this approach is to enhance the quality of Mrs. Smith’s life and to get her back to her previous lifestyle. The palliative care of Mrs. Smith will be approached in five major ways. Advanced care planning (ACP) will need to be done initially. This will involve her husband, the physician, the nurse and any other caregiver associated with her case.
