Population health management (PHM) was developed as a means of improving the health outcomes of a population by close monitoring and identification of individual patients within the group. They are used as intelligence tools that provide aggregate data as well as a comprehensive clinical picture of the population health. Healthcare provides, therefore, are able to track and improve clinical outcomes by lowering costs through the use of this data. A PHM program collates clinical information and makes it available for financial and operational statistics to provide actionable insights and improve inpatient care efficiency. It provides real-time actionable information to administrators and clinicians, enabling them to categorize and address care gaps within the studied patient group. Doing so yields better health outcomes and saves costs, especially among populations suffering from chronic diseases.
Problem Description
The African American population in the U.S. makes up about 13.5% of the country’s population (CDC, n.d.). Despite these numbers, it has been established that 42% of the new infections in 2019 came from this population, a figure that is substantially higher than that of whites and Hispanics (CDC, n.d.). A majority of those affected are male. Therefore, the population is a significant group to focus on as far as HIV/AIDS prevalence is concerned.
