As the nation puts more emphasis on transformation of the United States health care delivery system, nurses will always be recognized as playing a significant role in this strategy. There are the high expectations that the nursing jobs will increase and also become available in the American society and few will be available in the Emergency care hospitals.
The evolving practice of nursing care and Health care delivery practice ,how the practice of nursing care is expected to develop and grow and transform, the practice is anticipated to transform with regard to the practice, leadership and also the education framework in efforts to attain a higher function in provision of health care in United States (Hansen-Turton,2010).
The concept of continuity in nursing care is very important with regard to primary care especially when incorporated with other concepts like integration of health care, patient centered care and finally coordination of health care (Haggerty, 2003). Continuity of care or continuum of care can be described as the association that usually exists between a health practitioner and a patient, which will typically go beyond certain episodes of a given disease. In other words, the concept of continuity of health care is basically described in two main constituents: a clear focus on individual context and health care requirements of the patient; and also care over time, meaning the future, present and the past.
