Week 2
The long-term care service delivery system is a complex network of organizations, each with its challenges. To provide competent care and maintain quality, these organizations must work together to overcome their challenges.
The long-term care service delivery system consists of a wide variety of organizations, including the following:
- SNFs, including short-term rehab and long-term care skilled nursing facilities;
- ALFs, ILFs (Intermediate Care Facilities), CCRCs (Continuing Care Retirement Communities);
- Memory Care Facilities; Psychiatric LTC Facilities; Pediatric SNFs; Hospice Facilities; Respite Facilities; Developmental Disabled Facilities; LTC Mental Health Facilities.
Skilled Nursing Facilities: SNFs provide short-term rehabilitation and long-term care for individuals who need around-the-clock medical supervision and assistance with activities of daily living. They are staffed by licensed nurses and other medical professionals who provide 24-hour nursing care. SNF patients may recover from surgery or illness or have conditions such as dementia or Alzheimer's that require ongoing treatment and monitoring (Koopmans, 2022). SNFs may also serve as a transition from hospital to Home after a patient recovers from an acute illness or injury.
Assisted Living Facilities: ALFs provide housing and daily assistance with personal care needs to elderly residents who can live independently but need help with day-to-day tasks like dressing or bathing. Residents also receive meals on-site in a communal dining area and housekeeping services (Perracini, 2022). Most ALFs offer social programs such as exercise classes or trips outside the facility for residents
The main challenge includes;
1) Lack of standardization in the quality of services provided by each type of facility or setting.
2) Increasing demand for care that is affordable and accessible to all Americans.
3) Growing number of people with disabilities and chronic conditions who need support to live independently at Home or in an LTC facility.
4) Declining availability of family members who can assist in caring for their loved ones with disabilities or chronic conditions (Teaser).
