Abstract
The issue of medication error has increased its prevalence in various units of healthcare, and one of the most affected units is the surgical medical unit. Hence, there is a need to strategize on an intervention that will significantly reduce medication error issue in the unit. The paper has presented medication reconciliation as the appropriate approach to avert the situation. A 35% reduction in the rate of medication error is speculated within three months upon the implementation of the intervention. The implementation is done in three steps: Verification, clarification, and reconciliation. A quantitative and qualitative approach will be adopted in the analysis of the data based on the study. Linear graphs and tabulation will serve as the approaches and tools for data analysis. The success or failure of the intervention will be assessed based on various factors, including the number of medication reconciliation, number of correct medications, as well as engagement of patients in medication reconciliation. Ethical considerations will be taken into perspective, including the HIPPA guidelines and the reviews from the consent from the Internal Review Board institutional review board (IRB).
